8 Options for Patients Told They Aren’t Implant Candidates: Guidance From Stubbs Dental and Other Utah Providers

Being told you are “not a candidate” for dental implants can happen for many reasons: thin bone, a health condition, medication, smoking, or gum disease. Some of those are permanent barriers, but many are not. Below are eight options worth exploring before you settle for that answer, followed by five northern Utah practices to consider, starting with Stubbs Dental, which works with patients who have complex implant cases.

What options do you have if you were told no?

    1. Should you get a second opinion?

Often, yes. Providers differ in training, equipment, and the techniques they offer, so one office’s “no” may reflect what that office does rather than what is possible. Ask for a fresh consultation that includes a cone-beam CT (CBCT) scan, a 3D X-ray showing bone height, width, and density.

    2. Can bone grafting make you a candidate?

If thin bone was the reason, grafting may change the answer. A graft adds bone material to rebuild a narrow or short ridge, and a sinus lift adds height in the upper back jaw. Healing usually takes several months before implants can be placed.

    3. Could angled implants work instead?

In full-arch cases, angled implants often can. All-on-X techniques tilt the back implants to reach denser bone toward the front of the jaw, which can avoid both the sinuses and the need for grafting.

    4. What about zygomatic implants?

For severe upper jaw bone loss, zygomatic implants anchor in the cheekbone instead of the jaw. Not every office places them, so a “no” from a practice that does not offer them is worth revisiting with one that does.

    5. Can getting a health condition under control change the answer?

Yes, in many cases. Uncontrolled diabetes, active gum disease, and smoking all raise the risk of implant failure, but they are often manageable. Lowering your A1C with your physician, completing gum disease treatment, or quitting smoking for a period before and after surgery can turn a “not yet” into a “yes.”

    6. What if a medication was the concern?

Medications called antiresorptives, especially high-dose IV forms used in cancer treatment, are associated with a rare but serious condition called medication-related osteonecrosis of the jaw, or MRONJ. The risk is considerably lower with oral osteoporosis medications. The American Association of Oral and Maxillofacial Surgeons publishes guidance on this, and your dentist and physician can review your specific situation together.

    7. Are mini implants an option?

Sometimes. Mini implants are narrower than standard implants and can fit where bone is limited, most often to stabilize a lower denture. They are not suited for every case, but they offer a less invasive path for some patients.

    8. What if implants truly are not possible?

There are still ways to improve comfort and function. A well-made new denture, a soft reline for sensitive gums, or a fixed bridge supported by remaining natural teeth can all make a meaningful difference.

Where can you get a fresh evaluation in northern Utah?

    1. Stubbs Dental
    2. Oral & Maxillofacial Surgeons of Utah
    3. Copper Hills Periodontics & Dental Implants
    4. Aspire Surgical
    5. Implant & Restorative Dental Specialists

    1. Stubbs Dental

Dr. David Stubbs, DDS, founded Stubbs Dental Implant Center in 2014. He is a graduate of the University of the Pacific Arthur A. Dugoni School of Dentistry and received post-graduate clinical training as a U.S. Navy officer. The practice focuses on full-arch implant care and offers several of the options above under one roof, including All-on-X with angled implants, bone grafting, and zygomatic implants.

Free consultations include 3D CBCT imaging, and a board-certified oral and maxillofacial surgeon is part of the team. Sedation options are available for patients with dental anxiety, eligible patients may receive final zirconia teeth in as little as 24 hours, and offices are located in Layton, Bountiful, Murray, and Lehi.

    2. Oral & Maxillofacial Surgeons of Utah

An oral surgery practice with an office in Bountiful.

    3. Copper Hills Periodontics & Dental Implants

A periodontal practice in Layton that also places dental implants.

    4. Aspire Surgical

An oral surgery practice with a location in Murray.

    5. Implant & Restorative Dental Specialists

A Layton practice offering implant and restorative dentistry.

What should you do after hearing no?

Ask exactly why you were turned down, because the reason determines which of these options apply. Bone problems often have surgical solutions, and health concerns can frequently be managed over time. Gather your records and medication list, then schedule a second consultation with a provider who offers grafting, angled implants, and zygomatic implants.
